You only have a few things left to do . . .
Here’s the checklist of other documents to complete:
- Personal Statement: (at least 150 but no more than 300 words) describing why you want to attend Five Towns College. Submit your Personal Statement online here.
- Letter of Recommendation from a teacher, supervisor, guidance counselor or other non-related party. Student athletes-please submit letter of recommendation from your Coach. Scan, email or fax directly from this person to [email protected] or fax to (631) 656-3199.
- Official High School Transcript (an unofficial High School Transcript can be submitted for consideration until the official is received) or College Transcript(s) for Transfer Students. Please click here if you would like Five Towns College Office of Admissions to submit a Request for Official High School Transcript on your behalf.
- Pre-recorded Audition/Portfolio for Music or Theatre majors. Submit your pre-recorded audition/portfolio online here.
