This form should be completed as the first step for any current student, recognized Club/Organization, or Resident Assistant seeking to submit an event for consideration and approval. This form should be completed and submitted no less than four weeks prior to your event. Along with this form, we recommend that you include the following attachments:
A detailed budget request which includes each specific item requested, the store/website where the items will be purchased from, (include links) the price of each item requested, and an estimated total. (PDF Format, Max File Size: 5MB)
