The $35 application fee is waived when you Apply Online. You can fill out the online application here.

A Personal Statement of at least 150 but no more than 300 words is required. This essay can be uploaded and submitted to Admissions@ftc.edu via this link.

Official copies of high school and college transcripts should be sent directly from all institutions you have attended. The mailing address is Five Towns College, Admissions Office, 305 N. Service Road, Dix Hills, New York 11746-5871.

Meanwhile, you can submit an unofficial transcript temporarily but please follow up with the official transcript as soon as possible.

In addition, while it is the student’s responsibility to request and to timely provide Five Towns College with an official copy of their high school transcript to be evaluated for admission, in the event the student applicant needs assistance or to expedite the process, the student can give Five Towns College permission to request the student’s transcript on their behalf. If the student wants Five Towns College to assist, please complete the Permission to Request Transcript form below.

One Letter of Recommendation is required. In this regard, please submit a Letter of Recommendation from your guidance counselor, teacher, member of the community or your employer to support your application, not a family member. Student-Athletes should submit a Letter of Recommendation from their Coach. In addition, this letter should be scanned, mailed, emailed or faxed directly from this person to admissions@ftc.edu or (631) 656-3199.

Applicants for our Performance, Music Education, and Theatre programs are required to successfully complete a competitive audition.  Please contact our Admissions Office at (631) 656-2110 or admissions@ftc.edu to schedule your audition. In-person auditions are preferred. In limited situations, applicants may be granted permission to submit a virtual audition or a pre-recorded audition, however the permission of the Chair of Music/Theatre is required. Details are available on our Audition Requirements webpage. You can also  contact our Admissions Office for more information.

SAT/ACT scores are optional.

Applicants for our Music Education and Court Reporting programs are required to successfully complete an interview with program faculty.  Please contact our Admissions Office at (631) 656-2110 or admissions@ftc.edu to schedule your interview.

Early Action Incentive Grant

Important deadline!

Students applying for admission for the Fall 2026 must apply and deposit by December 1, 2025, to qualify for the Early Action Incentive Grant (EAIG) of $1500. The EAIG is no longer available for those applying for the Fall 2026; however, students applying for admission for the Fall 2027, should apply and deposit by December 1, 2026 to qualify for the EAIG. Proof of  Immunization

Finally, if you are accepted to the College, you will be required to submit proof of immunizations. Please read about the NYS Department of Health Requirements by clicking here.  The Department of Health regulations provide, in part, “This section discusses requirements for attendance at a post-secondary institution regarding immunization for measles, mumps, rubella and meningococcal disease in accordance with PHL Section 2165 (measles, mumps and rubella), PHL Section 2167 (meningococcal disease), and Title 10 New York Codes, Rules and Regulations Subpart 66-2 (10 NYCRR Subpart 66-2).”
